| 内容記述 | The term "community-based integrated care" denotes a system that comprehensively supports the daily life of members residing independently in a community familiar to them; the model was formed as a revision of the previously existing public long-term care insurance system that was primarily formulated for the elderly. Collaboration between various professionals is deemed a vital aspect of providing support. In recent years, the concept of a "society where people are harmoniously involved in the community" has attracted attention for its relevance to community-based integrated care. An emphasis is now being placed on the well-being of all individuals, including children, the elderly, and people with disabilities; a focus also prevails on the active engagement of residents within a community. This paper examines the appropriate method of employing a multi-professional collaboration in community-based integrated care by bridging the conceptual gap between community-based integrated care and a society where people are harmoniously involved in the community. In so doing, this paper bears in mind the importance of multidimensional well-being, which includes physical, psychological, and social aspects. To achieve appropriate multi-professional collaboration, the study also considers ways in which professional education should ideally be imparted. | |||||||||
