| 内容記述 | The objective of this study is to clarify the process by which health, medical, and elfareprofessionals support older adults with dementia living in regional communities. Semi-structured interviews were conducted with eight specialists who consented to participate, and the data were analyzed using the Modified Grounded Theory Approach. The results indicate that, in providing support to older adults with dementia, the specialists leveraged their understanding of regional communities and the trengths of local residents to implement a patient- and family-centered approach tailored to each patient’s condition and family context. Additionally, multidisciplinary coordination was utilized to evelop network-based solutions, alongside comprehensive support initiatives such as raising awareness about dementia among local residents. Furthermore, by identifying and enhancing patients’ strengths and fostering their capacity to receive support, more individualized aid can be provided, thereby broadening the scope of assistance offered. Finally, the results indicated that the emaining challenges can be addressed by understanding trends among individuals for whom providing support is difficult and by offering continuous interventions starting from the early stages of ementia | |||||||||||
